Transformation Assessment - Year 6
The Transformation Assessment is a final review task designed to check student understanding of the three main types of transformations: translation, reflection, and rotation. Students are asked to apply a specific transformation to six different shapes on a grid, using written instructions and rotation points provided.
This task supports formal assessment, group feedback sessions, or one-on-one conferencing, and includes translation, vertical and diagonal reflection, and rotations around fixed points. Answers are included for your convenience.

- We are learning to recognise and create tessellations of shapes, e.g. tiling patterns, describing the transformations used.
- We are learning to identify and use combinations of translations, reflections and rotations.

Modifications
• Provide transformation clues with visual scaffolding (arrows, angle cues).
• Use colour pencils to distinguish original and transformed shapes.
• Offer verbal explanations or teacher modelling for supported learners.
Extensions
• Ask students to explain the reasoning behind each transformation in writing.
• Provide incorrect transformations and ask students to identify and fix the errors.
• Challenge students to write their own multi-step transformation instructions.
