Game and Activity Assessment Record
The Game and Activity Assessment Record was created to provide a simple tool that allows teachers to track and measure student progress while playing educational games in Rotation Activities.
- Open the file. Type your list of student names down the left hand side of the sheet.
- Save this to your computer so you can use it for additional games.
- Then simply type the name of the game in the blue bar across the top of the page.
- Print this sheet and place it inside the front cover of the game. If your games are kept in zip lock bags, place the sheet face up inside the bag. If your games are kept inside clear plastic envelopes, place it in the front of the envelope.
- Students will pull out the sheet, add the date across the top and locate their name, using the colour guide to indicate their competence in this skill.
- Teachers can glance quickly at the page and easily locate those students who require additional assistance to learn the skill.
